APPLICATION TO COMMENCE THE PROFESSIONAL PHASE OF THE PHYSICAL THERAPIST ASSISTANT (PTA) PROGRAM

Instructions 2026

1. Ensure that you meet all eligibility criteria:
  • Earned a cumulative GPA of 2.5 or higher
  • Completed all pre-requisite courses within their listed timeframes (refer to expiration dates)
  • Earned the minimum grade required for each pre-requisite course
  • All seven required pre-requisite courses must be completed and located on the 糖心视频 transcript by the end of the spring semester in which an application was submitted
  • Attended a mandatory PTA Program Information Session prior to submitting an application
  • Not have been suspended or expelled from 糖心视频 or dismissed from a Health Professions program at Mercer
2. Prepare the initials/signature page:
  • Print it (PDF).
  • Read it.
  • DO NOT USE CHECKMARKS. Initial each statement.
  • Sign and date it.
  • Scan it into your computer.
3. Complete the online PTA application between April 1, 2026 and April 30, 2026:
  • Application is located on the right side of this page.
  • You will be asked to upload:
    • The initialed and signed initials/signature page
    • Unofficial transcripts from other academic institutions where pre-requisites were completed
4. Complete the Writing Skills Assessment Packet between April 1, 2026 and April 30, 2026:
  • Must be completed at the West Windsor Testing Center through their online appointment request form at: /student_services_testing.shtml
     for the Student Course Testing Request Form.
  • You must have a Mercer Student ID when you make your appointment.  If you do not have a student ID, you must first register to become an 糖心视频 student.

  • Instructions for completing the online appointment request:

    • 7 (Course): PTA
    • #8 (Test Number): Assessment
    • #9 (Professor): Beinert
    • #13 (Location): West Windsor Academic Testing Center (West Windsor - LB 215)
